Physical Therapy Initial Evaluation
We will start by actively listening to what’s been going on in your body. We will evaluate your musculoskeletal system to identify impairments and dysfunctions. From there we craft a tailored treatment plan that fits in your lifestyle. During your visit we provide targeted therapy to address immediate needs, while also developing your home program. It’s important to us that you are empowered with tools for ongoing care and progress outside of our sessions.

Physical Therapy Returning Visits
At your follow-up visits, we employ a diverse range of therapeutic modalities, including manual therapy techniques, neuromuscular reeducation, coordination training, strength building exercises, balance enhancement exercises, and other specialized rehabilitation tools. These interventions are meticulously tailored to alleviate discomfort, enhance function in your day to day life, and support you in achieving your unique goals. A standard plan of care generally spans 8-12 visits, though individual needs may vary, with some requiring more sessions and others requiring fewer.

Therapeutic Pelvic Yoga Sessions
Online or in-person, this therapeutic yoga session with Dustienne will help you develop a plan to feel stronger, increase mindfulness, create strategies around managing pain, and create a home program that supports you as you achieve your personal and professional goals.

Our practice is out-of-network with all insurance. This means your plan of care and allowed visits are not dictated by an insurance company.
Some insurance plans cover out-of-network physical therapy visits. Connect with your insurance company ask about your plan’s coverage.
Some clients have out-of-network physical therapy benefits and may receive partial reimbursement.
Some clients have in-network benefits, but have a high deductible that must be met before physical therapy is covered.
Some clients do not have physical therapy benefits and pay per visit regardless of if the clinic is in-network or out-of-network.
If you have out-of-network physical therapy benefits, you will be provided with a “superbill” for you to submit to your insurance.
Call your insurance and ask:
Are you required to receive pre-authorization prior to your initial visit/evaluation?
Do you need a physician referral, primary care referral, or a specialist referral?
What is your deductible? How much has been met? What percentage of reimbursement does your plan allows for?
Does the percentage change with a non-preferred provider?
Is there a cap to the amount of visits or money spent annually?
How does your plan ask you to submit? Is there a specific form they provide?
Verify the correct mailing address or fax number for claims submission.
Please understand that it is the client’s responsibility check benefits and understand that some plans do not reimburse.
Flourish Physical Therapy does not have a relationship with Medicare which means we are not legally able to treat Medicare recipients because of the patient protections laws that are in place. This might be my very first Google review but I'm so happy to write it for Alex Papale. About 6 weeks after severe swelling in my third trimester, a complicated birthing experience and c-section, and a tough physical recovery, Alex supported me through my postpartum recovery. They were so kind, so empathetic and so generous with their time and incredibly knowledgeable about all of the aspects of postpartum I and many people struggled with. I could barely change positions from laying down to sitting up without using support and experiencing pain. I couldn't stand without clenching my entire lower half of my body. I couldn't fathom looking at much less touching my c-section scar to start sensitizing and massaging it for proper recovery care. Alex shared exercises, did a full pelvic floor exam, and helped me with every single thing that I was struggling with and even helped me recognize other places in my movement and abilities that I hadn't recognized myself to support me through them. I would recommend Alex in a heartbeat. I don't know that I could get through my postpartum journey without them.

Dustienne has truly changed my life! After struggling with pelvic pain for years due to endometriosis, even after two excision surgeries, I never thought I’d know what it was like to live a pain-free life. Dustienne listened to me and curated a treatment plan to help me find relief. After working with her for about a year and a half, I’ve found true relief from my pain and now have the tools to manage flares on my own. Dustienne truly cares about her patients and I would recommend her to anybody who hasn’t been able to find relief from their pain.

I would highly recommend Alex Papale to anyone who is seeking pelvic floor or orthopedic physical therapy. They are trauma-informed, extremely considerate, knowledgable and kind. Dustienne truly gave me my quality of life back as I recover from Endo excision surgery.
